I hereby grant permission for to participate in the programs and sailing instruction given by Venice Youth Boating Association, Inc., including on the water training on bay or Gulf, in VYBA boats or sailor owned boats.

I, further release Venice Yacht Club and members of their Board of Directors and Venice Youth Boating Association, Inc., the members of their Board of Directors and their assigned instructors from any liability in the event of injury or accident, to the above named individual while on the premises of Venice Yacht Club or any other property used by the Venice Youth Boating Association, Inc. for sailing instruction or during any supervised sailing activity or race.

I fully understand the inherent risks involved in any sailing program, and with this knowledge, will allow my child, to participate in the program.

NOTE: Any child who behaves in such a manner as to endanger the safety of others or is continually disruptive during classes, on water or land, will be barred from further participation in all classes and instruction. Any child who repeatedly arrives late for class will be asked to leave for the remainder of the session. There will be no refund of any fees.
